Two proofs of residency (Driver's License, utility mail with name/address, lease, etc.)
Updated Kentucky Certificate of Immunization
Copy of student's social security card
Copy of state issued birth certificate
Other documents for the fall :
Kentucky Eye Examination form completed by an optometrist or ophthalmologist by Jan. 1, 2024
Kentucky preventative Health Care Examination Form dated after August 1, 2022
Kentucky Dental Screening Examination Form completed by a dentist, dental hygienist, physician, RN, ARNP or PA by Jan. 1, 2024
