• $85 Team Fee - Due By June 13th

• $40 per Player - No Food

• $50 per Player - Food Included

• 6 Game Guarantee & T-Shirt

THREE DAY INFORMATION:

• $95 Team Fee - Due By June 13th

• $50 per Player - No Food

• $60 per Player - Food Included

• 9 Game Guarantee & T-Shirt

CAMP INFORMATION:

• Coaches: Please bring physicals of each player when you check-in.

• Players: At registration please bring the following: Medical Release Form.

• THE ABOVE FORMS ARE MANDATORY FOR ANY PLAYER WHO PARTICIPATES IN CAMP. IF A PLAYER DOES NOT HAVE THE
  FORMS OR IF THEY ARE NOT FILLED OUT CORRECTLY, THE PLAYER CANNOT PARTICIPATE IN CAMP.
